feat: standalone MCP server (ra-h-mcp-server)

- Added apps/mcp-server-standalone/ with direct SQLite access
- Works without RA-OS app running
- Install via: npx ra-h-mcp-server
- Updated docs/8_mcp.md with setup instructions
